• PPO - Preferred Provider Orientation plans provide members in-network coverage with a higher level of benefits. These plans typically have the option of choosing out-of-network care at a higher out-of-pocket cost.
• EPO - Exclusive Provider Organization plans provide service coverage within the plan’s network, except for covered emergency care. Typically EPO plans use the same network as the PPO plans.
• HMO - Health Maintenance Organization plans provide a narrower network of health care providers. Coverage only services within the network except for covered emergency care. Typically, a primary care provider oversees member care.
